This site has been provided to allow your facility to identify all individuals involved in the frontline battle against COVID-19.
Please register using the link below with the email address you were contacted with. Once confirmed you should update your facility's details and then register all the frontline workers from your entity. You should also indicate which of these individuals you consider to have particularly distinguished themselves on the frontline of the COVID-19 response – those who have clearly gone above and beyond any professional expectations or call of duty.
For those who have sacrificed more than most, please highlight them as a "Hero", while for those exceptional individuals whom you consider to have sacrificed the most, please highlight them as a "Super Star". This category of "Super Star" is the most distinguished, and please limit this number to a maximum of 3 individuals in your facility.
For each individual you consider a "Hero" or "Super Star", please state why your colleague deserves special recognition: what they did; how they endured danger or hardship to serve others; what sacrifices they made; and how they inspire you.
